ChenMed was founded by Dr. James Chen and his wife Mary in 1985. Dr. Chen, a primary care physician, opened a small clinic in Miami, Florida, with a mission to provide high-quality, compassionate care to underserved senior patients.

SDOH are a major factor in our patient population. As a PCP, you are expected to:

  • Screen for SDOH regularly, including food insecurity, transportation, housing, and social isolation
  • Connect patients to appropriate community resources
  • Incorporate SDOH findings into the care plan
  • Recognize that addressing SDOH is directly tied to reducing hospital arrivals

All clinical team members are expected to:

  • Capture and manage all diagnoses for every patient proactively and continuously
  • Follow evidence-based medicine protocols and our Core Model
  • Prioritize lifestyle and behavioral interventions before medical ones, per our Service Standards
  • Collaborate closely within the PCP-led care team
  • Maintain strict compliance with CMS and Medicare Advantage regulations
  • Actively work to reduce hospital admissions

ChenMed follows evidence-based medicine. We do not chase the latest unproven treatments. Our clinical hierarchy of preference is:

  1. Lifestyle and behavior interventions first — diet, exercise, smoking cessation, and similar approaches
  2. Medical interventions when lifestyle approaches are insufficient
  3. Specialty or ancillary services only when necessary, with close PCP coordination

ChenMed applies a value-based approach to all clinical decisions: safety and efficacy come first, then cost. We weigh cost equally only after establishing that treatment is safe and effective. This approach protects our patients who often have limited financial resources.
