"After seeing him every week, after providing him door-to-doctor transportation, giving his medications on site, having conferences and coordinating care with his other specialists, we took him from being admitted five times per year – of which each admission costs $20,000 – to now, he has not been admitted in over four years, and he is out of hospice."
